As it turns out the culprit was Terminal Server.  The user was  
actually logged out but the session remained active with Outlook  
polling Exchange Server in the background. The user exceeded the 32  
session limit and Exchange subsequently refused any further  
connections. A reboot of the TS fixed the problem for now.  I'm not  
100% satisfied yet.
