The best practice I have used says to not use terminal services in
application mode on an exchange server and whenever I do install office in
app mode on a terminal server, I usually do not install outlook but rather
create an internet link on the desktop to OWA

As it turns out the culprit was Terminal Server. The user was actually
logged out but the session remained active with Outlook polling Exchange
Server in the background. The user exceeded the 32 session limit and
Exchange subsequently refused any further connections. A reboot of the TS
fixed the problem for now. I'm not 100% satisfied yet.

Save the user’s .PST and .nk2 files and delete the Outlook profile. When
you recreate and verify connection, copy these back to their original
location.

I have one client (of 30) that will not connect to Exchange Server. The
user has had no problem for almost two years and now, last Wednesday, the
client refused to connect to Exchange. I created a second user on the
computer and connected to exchange with no problem. She gets mail via her
iPhone so I don’t believe her account is the problem.
